Fraternity/sorority organizations must complete the following forms by the due date identified or when applicable.
Fraternity/Sorority Organization Information Form
Provide general organization information, officer contact information, and advisor contact information. This form is located on CatLife and can be accessed through the Fraternity and Sorority Life page under "Forms."
Philanthropic Donation Report Form
Organizations are encouraged to record and submit all philanthropic donations using this form. Only submissions to this form will be considered for The Greek Initiative.
Fraternity/Sorority Roster Update Form
All membership status updates to the organization's roster should be submitted here within five days of the change.
Program/Event Reporting Form (New form coming soon)
As per the requirements included in The Greek Initiative, please report the outcome of programs and events your fraternity or sorority coordinated, hosted and/or attended.
Campus Involvement Log (New form coming soon)
Use this form to upload a list of members with campus involvement and position(s) held, if applicable. Download template.
Campus Employment/Internship Log (New form coming soon)
Use this form to upload a list of members with campus department and employment/internship position(s) held, if applicable. You can download the template.
Upload Miscellaneous Documents (New form coming soon)
Provide chapter documents on this page. These documents include chapter goals, new member education program, scholarship plan and a previous scholarship plan summary.
